If you have a chronic health condition — a problem that has been ongoing for more than six months — speak to your Donnybrook Family Doctors GP. We can tailor a chronic disease management plan that suits your condition.
What's included
- GP Managed Care Plan with allied health (dietician, physiotherapist, podiatrist, chiropractor, diabetes educator, osteopath)
- Medication review and renewal
- Regular monitoring of bloods and observations
- Specialist coordination
Eligible patients receive part of the allied-health fees back through Medicare.
